The priority areas were identified through a lens of addressing social determinants of health using upstream approaches when possible. Encompassing a whole child-family model, we identified four priority areas. |

Strategic Thinking Using Data & Stakeholder Input | Committed to a community-driven health improvement process, Santa Cruz County used the Mobilizing for Action through Planning and Partnerships (MAPP) model for the health improvement planning process. Developed by National Association of County and City Health Officials (NACCHO) and the Centers for Disease Control and Prevention (CDC), MAPP is a community-wide strategic planning process for improving community health. MAPP is a guide that helps communities apply strategic thinking, using data and stakeholder input to prioritize public health issues and identify the resources needed to address them. Involving both local public health officials and community stakeholders, the Santa Cruz County MAPP process launched in 2015, yielding both a Community Health Assessment (CHA) (published in 2017) and the subsequent Community Health Improvement Plan (CHIP) (published in 2018). |
