Drinker's Check-up is designed to help problem drinkers reduce their alcohol use and alcohol-related consequences.
Study participants had a significant reduction in alcohol use, alcohol-related consequences, symptoms of alcohol dependence, and a decrease in ambivalence about reducing alcohol use.

The overall goal is to reduce the prevalence of diabetes and improve the care of people with diabetes by improving provider education.
The results indicate that a half-day site visit with an experienced diabetologist can lead to sustained, improved glycemic and lipid control in previously-uncontrolled diabetic patients. The online iDose tool provides an easy way for healthcare providers to calculate insulin dosage.

To combat the epidemic of violence among Baltimore’s youth and support traditional public safety strategies using a combination of public health and human service models to reduce violence.
It was estimated that the program was associated with 5.4 fewer homicide incidents and 34.6 fewer nonfatal shooting incidents during 112 cumulative months of intervention post observations.

The goal of the Place Matters initiative is to improve the health of participating communities by “addressing social conditions that lead to poor health” through “identifying the complex root causes of health disparities and defining strategies to address them.”
AC Place Matters has identified 10 best practices that could help other programs address social determinants of health: find and foster strong leadership, dedicate staff resources to the work, engage staff from across the local health department, contribute to building grassroots power, address root causes, partner with community organizations and leaders, partner with government institutions across sectors, work reactively and proactively, build capacity, and use tools that ensure a focus on health equity.
