Goal: To use tai chi exercise to improve balance and decrease incidence of falls among older adults.

Impact: The program shows that ta chi can significantly improve health-related outcome measures in older adults and such a program can be practically and effectively implemented and maintained in community settings.

Goal: The Ambulatory Integration of the Medical and Social (AIMS) model aims to address social and environmental factors patients face that may prevent them from following their plan of care, thus impacting their health.

Impact: The AIMS model helps create better supported, less stressed, and better informed consumers and caregivers. There is also evidence to suggest that this model reduces ED usage and 30-day readmissions in participants.

Goal: The goal of the Winning With Wellness (WWW) project is to promote physical activity and healthier eating habits in order to reduce and prevent childhood obesity.

Impact: School wellness programs that are acceptable to teachers and utilize some already existing resources can create impact for rural youth by improving nutrition offerings in school and increasing physical activity during the school day.

Goal: The goal of the Young Parents Program is to help inner city teenage parents learn positive attitudes, behaviors, and parenting skills so that their children experience healthy growth and development a during their early and critical years.

Goal: The goal of the Al's Pals program is to teach children how to practice positive ways to express feelings, relate to others, communicate, brainstorm ideas, solve problems, and differentiate between safe and unsafe substances and situations.

Impact: Studies have shown that the program resulted in higher degrees of positive change in the intervention groups, increases in prosocial behaviors and positive coping behaviors, and decreases in antisocial and negative coping behaviors.

Goal: Joslin's Asian American Diabetes Initiative has a mission to enhance the quality of life and health outcomes for Asian Americans living with diabetes through research, education, outreach and culturally appropriate treatments.

Impact: Joslin's Asian Clinic provides comprehensive and culturally appropriate diabetes care for Asian Americans.

Impact: The Community Preventive Services Task Force (CPSTF) recommends school-based asthma self-management interventions to reduce hospitalizations and emergency room visits among children and adolescents with asthma. Evidence shows interventions are effective when delivered by trained school staff, nurses, and health educators in elementary, middle, and high schools serving diverse populations.
When implemented in schools in low-income or minority communities, interventions are likely to promote health equity.

Goal: The mission of the ATHENA (Athletes Targeting Healthy Exercise & Nutrition Alternatives) program is to promote healthy sports nutrition and discourage the use of body-enhancing substances among middle and high school female athletes.

Impact: Participation in the ATHENA program results in significant reductions in the use of performance-enhancing substances, recreational drugs, diet pills, tobacco, and alcohol among female teen athletes. Healthier eating and other health behaviors, and body image perceptions were also improved.

Goal: The partnership uses a comprehensive strategy with four specific goals:

1. Carry out a multi-agency law enforcement (suppression) strategy to reduce gun-related and other violent crimes committed by youths 17 and older.
2. Operate an intensive intervention program to reduce the risk factors for the highest risk youths, their families, and the community.
3. Mobilize the community at the grassroots level to address the problems of hard-to-reach families and the highest risk youths.
4. Operate a long-range prevention program that identifies, links, and strengthens existing resources to serve youths who may be at risk.

Goal: The Be a Star program was developed to help preadolescents gain the knowledge and skills necessary to resist drugs.

Impact: During the third year of the evaluation, very strong differences emerged between intervention and control groups. The treatment groups scored significantly higher on the scales rating family bonding, pro-social behavior, self-concept, self-control, decision-making, emotional awareness, assertiveness, cooperation, attitudes toward drugs and alcohol, self-efficacy, attitudes toward African-American culture, and school bonding.
